I am attempting to make a selection form. In the form I would like to allow the user to check their favorite songs from a list, then make a comment with that selection. So, there would be a checkbox, the song title, a line from the song, then a textbox that would allow a comment such as uplifting, inspiring.....
I can make the checklist but cannot embed the text box. I can make a single checkbox, then a comment box underneath, however the two are not tied together when the email is submitted.
Any help would be great.
