Okay. Let me ask a few questions.
1. Have you ever created a form successfully with CF7?
2. Have you tried using the WP default theme (Twenty-Eleven, I think)?
3. This is a stretch, but have you tried creating a form in English? I only say that just in the odd chance that there a language issue. There shouldn't be a problem using a language other than English, but you never know.
