I checked your questions and finally I found _my_ bug! I had modified the post-template.php in wp-includes, because I had the strange effect to see the content from a page twice. I undo my changes and the form appears, but now my former problem is back again: the content comes twice :-(
It seems the be a problem with an other plugin. When i deactive this plugin, the content comes only once and the form is shown. So I have to search in an other "code".
