lolaludesign on "[Plugin: Contact Form 7] Check boxes not showing up."

I've created a form with Contact Form 7, however, the check boxes are failing to show up on the page (see http://renewalbydesign.com/contact/). There should be check boxes in front of the items under the services sections. Any ideas of how to fix this?
