fourjandals on "[Plugin: Contact Form 7] Email not showing all the fields"

I am trying to develop a custom Contact Form on this page:
http://www.scandycollection.com/ordering-form-for-men/

It looks ok on the site and it does send an email. However, the problem is that it doesn't send any of the data from the custom fields I have created. It only sends the Name, Email, Subject and Message fields.

Why is this?
