Acceptance checkbox not checked in Dorayaki theme (it works though)

Hi, please help: The acceptance tag works nicely with acceptance_as_validation: on, but the box is not on line with the text and more importantly it does not appear to be checked even if clicked. It works though, you just can’t see that it is checked, confusing people. What can I do?

Code is:
